Location: Brooklyn/Queens, and Manhattan


Hours: (TBD)


Salary: $100K-119K plus $15,000 Sign-on Bonus or Student Loans Assistance, paid over the course of 3 years



 

 

We have 2 open roles for Coordinator of Care Nurses. In this role, you will assume the ongoing, primary responsibility to coordinate, implement and continually evaluate the home care needs of your patients. You will also review patient insurance information, coordinate activities involved in each individual care plan and make sure that pertinent findings are shared with family and professional caregivers. Your excellent communication, documentation and time management capabilities will be essential to your success. And all along the way, you will experience the respect for your opinions and the high level of autonomy you need to do the job right.
Required Qualifications :


  • NYS Registerd Nurse license

  • Minimum of one-year acute, Medical-Surgical nursing experience; CHHA/LTHHCP  experience preferred

  • Associates degree in Nursing required; Bachelors preferred

  • Bilingual skills (Spanish, Cantonese or Mandarin)

  • Customer Service - Strives to provide the best customer service at all times to internal and external customers.

  • Communication - Utilizes appropriate communication methods and style to ensure information is efficiently and effectively delivered and received.

  • Collaboration/Cooperation - Works well with others and takes actions that are most appropriate for achieving departmental and organization goals

  • Dependability/Accountability - Meets productivity, attendance and punctuality standards

  • Professionalism - Demonstrates MJHS core values, adheres to the MJHS Code of Conduct and Standards of Conduct

  • COVID vaccination is required

  • Driver license and car required for Brooklyn and Queens region


  • Candidates are required to pass a pharmacology test as part of the recruitment process
