Title: Social Worker, Hospice (4)
Position Type: Permanent, Full-time
Agency: New York City health system
Location: Bronx, Queens, and Manhattan
Hours: (TBD)
Salary: $68K-82K plus $15,000 Sign-on Bonus or Student Loans Assistance, paid over the course of 3 years
We have 4 roles for Social Workers, who provide psychosocial support counseling and other social work-related services to Hospice and Palliative Care patients and their families in the community homecare setting and refers to hospice whenever appropriate.
Required Qualifications :
- Social Work license (LCSW), Palliative Care Certificate, Pediatric Specialty
- 3 years of experience in Social Work home care or hospice preferred
- Social Work Degree
- Bilingual skills (Spanish, Cantonese or Mandarin
- Customer Service - Strives to provide the best customer service at all times to internal and external customers.
- Communication - Utilizes appropriate communication methods and style to ensure information is efficiently and effectively delivered and received.
- Collaboration/Cooperation - Works well with others and takes actions that are most appropriate for achieving departmental and organization goals
